    At Kindred, communication and collaboration with our medical providers is an important part of improving our quality, service, and customer satisfaction. Kindred has been a leader in bringing a wider breadth of medical services, specialty physicians, and new practice models into the post-acute setting. As the volume and complexity of post-acute, transitional care patients has increased, so has the need for a more diverse set of medical professionals. Where it would have been a shock to see a pulmonologist or plastic surgeon in a SNF when I started my training, this is now routine. But because so many physicians are so new to the setting, this creates new challenges as well.

    This website is designed to provide all medical providers in Kindred SNF’s the tools and information they need to practice effectively, fulfill their role as leaders in the SNF environment, and improve quality and compliance for their SNF patients. With SNF quality and accountability on the minds of patients, payors, and regulators, understanding the roles and responsibilities of the medical provider in the SNF culture is critically important. Kindred is committed to working with our providers, helping them demonstrate meaningful outcomes, supporting their various practice models, and coordinating care across the continuum.
